APPLICATION REQUIREMENTS

Metro Manila applicants shall submit the following requirements to the DILG-MIMAROPA Regional Office addressed to:

                                        FLORIDA M. DIJAN, DPA, CESO III

                                        Regional Director

                                        DILG-MIMAROPA

                                        822 Quezon Avenue, Brgy. Paligsahan,

                                        Quezon City

                                                    Attention : RPSPB Secretariat

1.          Application Letter indicating the position applying for

2.          Personal Data Sheet (2017 CSC Form 212) with latest picture

3.          Photocopy of the following:

a.          Transcript of Records

b.         Diploma

c.          Photocopy of Certificate of Eligibility/ Board Rating/ License

d.         Photocopy of Certificate/s of Training Attended/ Participated relevant to the

application being applied

e.          Non-academic Awards received within the past 12 months

f.           Performance Rating for the last two rating periods (if applicable)

** Provincial applicants shall submit their complete application requirements to the nearest DILG Provincial/ City Office for proper endorsement to the Regional Office.
